Community rehabilitation team

The team is made up of occupational therapists, physiotherapists and support workers who are experts in developing rehabilitation programmes for patients within the community.

There are four teams working in each neighbourhood across the borough providing treatment in patients own homes:

  • Old Trafford, Stretford – Based at North Trafford Combined Care Centre. Telephone: 0161 860 2703

The fifth team provides an outpatient service for patients (Seymour Outpatient Rehabilitation team). It is based on the Seymour Unit at Trafford General Hospital (telephone 0161 746 4570)

Hours of work

From 8.30am to 4.30pm, Monday to Friday.

The service

Patients referred for community rehabilitation receive individual treatment programmes to address their needs.

Programmes are aimed to last for up to six weeks, depending on the nature of the functional problem being addressed.

Where possible, patients will be referred to supporting services on discharge from intermediate care.

Once patients' needs have improved to the extent that they can travel to an outpatient appointment, they may be given the opportunity to attend a group exercise programme run by the team or attend the outpatient rehabilitation service at Trafford General Hospital.  

Please note that the response time from receipt of referral with the team to initial assessment will vary depending on the patient’s rehabilitation need. The response time will be agreed at triage through discussion with the duty clinician and the referrer.
