The heart failure nursing team aims to deliver a high-quality, streamlined heart failure service, helping each patient manage their illness as it progresses. The team actively seeks out people who have, or are at risk of developing, heart failure, but who have not been previously diagnosed. Working proactively helps the team reduce patients' risks and improve the support they receive.
Both heart failure specialist nurses provide a nurse led service where, as independent prescribers, the team initiates and titrates medication to improve the condition of heart failure patients, either in clinic, or in the patients home where necessary. The team also provides education to enable patients to manage their condition.
