Community matrons manage patients with long term conditions in their homes. The team works in conjunction with other health and social care professionals and the voluntary sector. It is attached to the Vanguard Practices and isbased at the Delamere Centre, Mitford Street, Woodsend, Chapel Road and Firsway.
The matrons aim to reduce unplanned hospital admissions and attendances at the accident and emergency department, and to enable patients and their families and carers to maintain their health and wellbeing for as long as possible, and address exacerbations promptly.
They are advance practitioners who can assess, diagnose and treat patients as independent prescribers.
