The tissue viability service is based at Partington Health Centre, and works closely with all district nursing teams in Trafford.
The tissue viability service offers specialist advice, support and assessment regarding skin viability to the residents of Trafford, their informal carers and to any health and social care professionals involved in their management, in any care setting.
The team promotes evidence-based best practice through the development and implementation of specific training and education packages.
They continually review national and international tissue viability related evidence and guidance, incorporating this into local policy to support evidence-based practice at a local level. The team supports NHS Trafford in achieving its clinical governance agenda by actively participating in risk assessment and management.
The leg ulcer service provides patients with a full holistic assessment, differential diagnosis and individualised management plan. Currently, any patients referred into the service who are genuinely housebound will be assessed at home by the leg ulcer nurse specialist, who will provide specialist advice and support around ongoing management to the primary care provider.
The specialist tissue viability nurse provides support, advice and clinical care in all aspects of wound management (including leg ulcer clinics and providing people with the equipment they need to look after their wound).
